Assessment Route

Step 1

Contact your District Nursing Team within your Health Centre, or the Adult Social Care Enquiry Team within the council and request an Equipment Assessment.

The quickest way to do this online via Request Support from Adult Social Work

 

Step 2

An assessor will undertake an assessment of need and request any equipment required. If you have to pay contributions towards your equipment, this will be discussed with you at assessment.

 

Step 3

The Community Equipment Store will provide equipment as soon as possible after it has been requested, and in line with Service Standards.

 

Step 4

A colleague from Health and Social Care will check in with you once your equipment has been delivered to ensure it is helping you as expected.
