This service is delivered by Carers of West Lothian in partnership with West Lothian Council.

What is an Adult Carer Support Plan or Young Carer Statement?

If you provide unpaid care for a family member, friend or neighbour, you have the right to request an Adult Carer Support Plan or, if you are under 18, a Young Carer Statement.

The plan helps you talk about your caring role, how caring affects your day-to-day life, and any support that could help you achieve your personal goals and maintain your health and wellbeing.

How do I apply for an Adult Carer Support Plan?

Carers of West Lothian will work with you to complete your Adult Carer Support Plan. The referral form will encourage you to consider what your needs are to ensure you get the most of completing a plan.

How do I apply for a Young Carer Statement?

Carers of West Lothian will work with you to complete your Young Carer Statement. Fill in your details on the Carers of West Lothian site and they will be in touch to help you complete your Young Carer Statement.

What happens next?

Carers of West Lothian will discuss your Plan or Statement with you to decide what support might be beneficial for you.

The process is outlined below:

  1. Request an Adult Carer Support Plan or Young Carer Statement.
  2. A worker contacts the carer.
  3. The assessment conversation takes place.
  4. Eligibility is considered.
  5. Support options are agreed.
  6. The carer receives a copy of their plan.

The worker will help you to plan your support - this may include:

  • Information and advice
  • Information about housing, equipment and adaptations
  • Technology like flood/heat detectors, wandering devices, home security and Careline support
  • Welfare benefits advice
  • Short breaks from caring
  • Who to contact in an emergency
  • How to arrange an assessment of need for the person you are supporting
