District Nursing
District nurses are qualified nurses who have undertaken a further graduate specialist practitioner programme.
They lead and support a team of qualified and unqualified nurses to deliver care in a patient's own home.
The nature of districting nursing has changed to meet the needs of patients in the community.
Complex care once only delivered in acute (hospital) settings is now being provided by district nursing teams in collaboration with key partners i.e. GP's, West Lothian Council, other healthcare providers and voluntary sector organisations such as Food Train and Carers of West Lothian.
The district nursing team have the skills and expertise to provide assessment and care to patients when illness, injury or weakness affects a person's ability to cope at home independently and safely.
